Entry
Enter your email address into the optional "sweepstakes" field for a chance to win one (1) of two cash prizes valued at $500 each. Entrants can win one of two potential cash prizes. No purchase necessary to win and a purchase does not increase your odds of winning.
Limitations
Only one entry per household. Must be a legal resident of the United States and 18 years or older and must enter by providing their email address in the sweepstakes entry form on ezbabyshop.com. Employees or agents of ezbaby, its affiliates, advertising/promotional agencies, and the members of their immediate families or those persons living in the same household of such individuals are ineligible to enter or win. All federal, state and local laws and regulations apply. Void where prohibited by law.
Winners
Two (2) winners will be randomly selected by a drawing conducted on 10/2/2022 at 10:00 AM PT from all opt-in entries received on 09/08/2022 at 8:00 AM PT through 09/30/2022 at 11:59 AM PT. There will be two (2) winners who will receive one (1) $500 cash prize. At the time of contest entry, entrants will enter their email address, name and phone number into the requested fields below this, on ezbabyshop.com. Odds of winning based on the number of entries received. The winners will be notified by email no later than the end of business 10/2/2022 and will have 24 hours from date and time of notification to respond to ezBaby, complete the claim form and other documents as necessary, and provide them to ezBaby. Should a winner fail to comply with these requirements or otherwise be unable to accept the prize, the winner will forfeit the prize and it will go to the first selected alternate. For a list of prize winners, mail a self-addressed stamped envelope to ezBaby, Attn: Myron Katsapas, Owner, 3639 Ridgeview Blvd, Wenatchee, WA, 98801. Any and all taxes assessed on the prize will be the responsibility of the winner. In the event of non-compliance with these requirements, the winner may be disqualified and an alternate winner selected.
Conditions
By participating, entrants agree to these Official Rules and the decisions of ezBaby, which shall be final and binding in all respects. By participating, entrants release ezBaby its respective affiliates, advertising/promotional agencies, and the employees, officers, directors and agents of any of the above organizations from any and all liability for any injuries, losses, or damages of any kind caused by participation in the Sweepstakes or the acceptance, possession or use/misuse of a prize or participation in any prize-related activities. Where legal, by accepting prize, winner grants permission to use his/her name, address, likeness, photograph and statements by ezBAby for advertising/publicity purposes without further compensation.

Do I have to report the prize to the IRS?
The IRS requires a recipient of any prize to report the value of the prize on their income taxes. This includes federal income tax, and some states require state income taxes.
If that prize is valued $600 or greater, then the sponsor of the sweepstakes or contest is responsible for issuing an IRS 1099-Misc. income tax form to the winner and the IRS.
This can be tricky because you need to obtain the winner’s Social Security number in order to file the form.
We recommend that you always make potential winners aware of the income tax responsibility and require them to formally accept the prize and provide their SSN or a W-9 in order to be declared a winner and before sending a prize.
